Treatment-Resistant Depression (TRD)
Treatment-resistant depression (TRD) is defined as a form of major depressive disorder in which an affected individual does not respond adequately to at least two different antidepressant medications administered at an adequate dose and for an adequate duration. In clinical settings, an inadequate response is most commonly defined as a reduction of less than 25% in depressive symptoms following treatment with an antidepressant.
The prevalence of treatment resistance is significant, with nearly 30% of individuals diagnosed with major depressive disorder being classified as treatment-resistant under current definitions. The condition may also be specified by the particular medications to which a patient shows resistance (e.g., SSRI-resistant) or staged based on the total number of treatments attempted.
Several factors can contribute to an inadequate response to treatment, including:
- A history of repeated or severe adverse childhood experiences
- Early discontinuation of treatment
- Failure to consider psychotherapy and other psychosocial interventions
- Patient noncompliance
- Misdiagnosis
- Cognitive impairment

Despite the challenges associated with treatment resistance, various effective options are available. These include electroconvulsive therapy (ECT), ketamine and esketamine, rTMS, and certain adjunctive agents.
There is ongoing debate among clinicians and researchers regarding the construct validity and clinical utility of treatment-resistant depression as it is currently conceptualized. A specific point of contention is whether inadequate responses to other treatment modalities, such as psychotherapy, should be included in the formal definition of TRD.
